I've had these inserts drop out on my 3L. Put them back using locktite about four years ago & it seems to have done the job. First time I've ever come across inserts like this.

Just a tip that saved me once: if the bleed nipple is u/s but removable (I drilled mine out!) a small ball bearing followed by a bolt will seal perfectly. Yes I know.....you still need a suitable ball & screw, but I can't do everything for you LOL.
