Turn the spare wheel bracket over, will lift the lot up more,
I think there must be 2 types of spare wheel carrier, as both of ours made no difference to the height when I turned them over, but I noticed it means you loose the drain groove that stops water sitting in the bottom of the carrier against the door.

I got a bit of 3/8" checker plate Ally that is 3 inches taller than the spare wheel carrier. I used the mount as a guide, and drilled 8 holes in the Ally, 4 with the mount at the top, and 4 with it at the bottom. The spare wheel carrier bolts into the 4 top holes using new nuts and bolts, and the bottom 4 holes bolt to the door. You will need a spacer between the plate and door to allow for the head of the bottom bolts that hold the carrier to the plate, so may need longer bolts.

You could do something similar with 2 lengths of box section as well.
