I have no skill with visual art, but fortunately I have a couple of friends who are very good. So I choose an image of theirs, get their permission to use it and sometimes help to change it slightly, then get a copy and do the sizing, effects and textwork 'freehand' using Arcsoft Photostudio. I then run the final jpeg through a PDF converter for the print cover, or just scale it for the ebook.

Oops, the important bit: both of my friends are 3D render/CGI artists. They use DAZ Studio with a lot of the add-ons.
