As header, the replacement bolt has come out and obviously need a new one. New one required.
 
Will look in workshop tomorrow, Zac did you tighten it up nice and tight, if so then could be the thread in the bell housing is getting slack, Rick

Received and installed, thanks Rick. :thumbs
What a pig of a job, Now that I've worked a way of supporting the starter motor (wood wedged underneath at the front), locating and cartching the thread becomes a lot easier. We're up and running again.
Obviously I'll keep an eye on it, it is a tight as I could get with a spanner.

I deffo spannered those in hard!!!!!!

I reckon you've got thread wear so keep an eye on it mate
