My shout would be fuel filter first, if that still doesn't resolve the issue then I'd be attempting to check the mini filter in the injector pump (if one is present) also depending on what injector pump is fitted.

I had a similar scenario with my Terrano whereby when I came to stop, the car would idle normally for a split second and then just start hunting, blipping the throttle cured it for a few seconds and then again it would return.

Once I removed the mini filter out of the injector pump the problem went away.

my advice before you go diving in the deep end is to give it a good service.
Air,oil and fuel filters,oil change and put a bottle of injector cleaner in it.
Clear the codes,go for a drive and see how it fairs up.
If the problem comes back thats when you can start looking more in depth at things
